BEFORE YOUR FIRST VISIT
NEW PATIENT FORM
Prior to your first visit, we ask you complete our new patient form. This provides us with more information about your health history and reason for seeking care. The more information we have, the better we can apply precise techniques that are customized for you.
Please download the form which is a PDF file in the link below. You will need Adobe Reader ® to view and either complete the form digitally and email to our centre (link below) or print and bring it with you on your visit.
If you don't have Adobe Reader ® installed on your computer, you can CLICK HERE to download it.
If you are unable to complete this form before visiting our centre, we simply ask that you arrive 10-15 minutes prior to your scheduled appointment time, and our staff will be happy to provide it for you when you arrive.
YOUR FIRST VISIT
WHAT TO WEAR
It is best to wear non restrictive clothing which allows for ease of movement.
Your first visit can last from half an hour or more depending on the injury or condition you are there to treat. Subsequent visits are usually shorter.
We will start by giving you an exam to check your reflexes, joints, hip flexors, spine, neck and posture. We may need to twist your body in different ways to check your range of motion. We may press on areas and ask if it hurts to help identify the problem.
During your exam we will discuss the extent of your injury or irritation, your general health, and most importantly what your goals are for your health.
We may give you an adjustment where the chiropractor uses their hands or a small instrument to apply a controlled, sudden force to a spinal or other joint. The goal of this procedure is to restore normal joint function and normal range of motion and reduce joint irritation.
We may give you exercises to do at home, recommend hot/cold therapy, and recommend if you need follow up visits.
HEALTH & NUTRITION
Chiropractors practice the concept of holistic medicine, which provides care for the entire body instead of concentrating on a disease’s symptoms. Part of holistic medicine includes nutrition, which uses food to bring health and wellness to the body.
ROLE OF SUPPLEMENTS
Unfortunately, most people don't get the recommended amount of important nutrients from food alone. Supplements can bridge this nutritional gap. This will enhance the nutrient density of your diet and make sure you are obtaining the right amount of nutrients tailored to your dietary needs. We can suggest foods and supplements that can greatly enhance your health as part of our holistic treatment approach.
We direct bill to Autopac, Worker's Compensation and most insurance companies including Blue Cross, Canada Life and Sunlife.
